Applicant/Head of House Identity

We may ask for the following information:

• Full legal name

• Age

• Proof of identity via ID or birth certificate

• Proof if income (if needed)

Contact, Location & Service Coordination

We may ask for the following information:

• Current living address

• Mailing address (if different)

• Primary phone number (if needed)

• Email address

• We may ask for proof of address depending on age

Household Composition & Age Verification

We may ask for the following information:

• Total number of individuals

• Name & DOB of each child (if applying for family help)

• Relationship to applicant

• Currently grade level for children

• Proof of education enrollment

Program-Specific Eligibility Requirements

We may ask for the following information:

• Utility type & provider

• Service address

• Disconnection notice

• Total past-due amount

Diaper & Infant Essentials

We may ask for the following information:

• Child's age & weight

• Preferred diaper size

Transportation & Clothing/Gear Aide

We may ask for the following information:

• Primary need justification

• Vehicle access/ownership proof

• Clothing size

• Shoe size
